All students are required have a valid Kentucky Immunization Certificate on file at school at all times. (refer to 902 KAR 2:060 for details, exceptions and exemptions). This includes all series and boosters for Diphtheria, Pertussis/Tetanus, Polio, Measles/Rubella, Mumps, HIB (Haemophilus Influenzae Type B), Varicella (Chicken Pox) and Hepatitis B. Dates of all immunizations (series and boosters) must be documented on the certificate. An expiration date stating when the next immunization is due must be indicated.

Initial Entry

  1. A valid Kentucky Immunization Certificate.
  2. A physical examination done within one year prior to entering school.
  3. Eye examination done by ophthalmologist or optometrist by January 1 of the year of enrollment.

Immunization requirements if child is age 4-6:

  1. 1 MMR and 1 Measles containing vaccine, on or after the first birthday.
  2. 3 Hepatitis B immunizations.
  3. 1 HIB immunization if under age 5.
  4. 1 Chickenpox vaccine after the first birthday, or a written statement by parent or physician that child has had the disease.
  5. 4-5 DTaP, DT, DTP, or a combination of the two, if the fourth dose was given on or after the fourth birthday, the fifth dose is not required.
  6. 3-4 OPV or IPV or a combination of the two, if the 3rd dose was given on or after the fourth birthday, the fourth dose is not required.

Sixth Grade Requirements :

  1. A physical examination done within one year prior to entering sixth grade.
  2. A valid Kentucky Immunization Certificate including one MMR and one Measles containing vaccine, after the first birthday.
  3. 3 Hepatitis B given over a 4-6 month time span, or an alternate 2-dose series consisting of two 10-mcg doses of Merck’s RecombivaxHB separated by 4-6 months.
  4. 1 Tdap or Td at age 11-12 if it has been at least 5 years since last dose.

Out of State Transfer Students :

  1. A valid Kentucky Immunization Certificate.
  2. A physical examination done within one year prior to entering school.

Tuberculosis Skin Test:

  1. A tuberculosis skin test must be given and read prior to enrollment in school if the student comes from Asia, Africa, the Middle East, Latin America, or countries with endemic tuberculosis.
  2. If the skin test is positive, the child must have a chest x-ray and medical examination prior to enrollment.

Meeting Requirements:

  1. Contact your school nurse or health care provider for an appointment as soon as possible. The services at the school health clinic are free.
  2. Take all personal immunization records with you.
  3. Take required completed forms to your child’s school office.

There are several other vaccines that are recommended and may prevent certain types of infectious disease but are not currently required for entry into schools or daycares. They are: Influenza Vaccine, Meningococcal vaccine, Human Papillomavirus (HPV), Pneumococcal vaccine, Hepatitis A and Rotavirus vaccine. Please contact your local health care provider, school nurse or the local health department for more information.
